All applicants for licensure must complete the form and submit it with the $135 licensure and first registration fee directly to the Office of the Professions at the address at the end of Form 1. The checks should be made payable to the New York State Education Department.

NOTE: The cancelled check is the student’s receipt.

The student has to answer all questions and provide all information requested. Failure to will delay its review. A Notary Public must notarize the student’s signature on Form1. Once the applications has been approved the student would be sent is an acknowledgement letter.

If the students are applying for licensure as a licensed practical nurse, they must submit a copy of their high school or secondary school diploma or transcript in the original language with their Form 1.

If the applicant were educated outside the U.S., he would have to submit a copy of his nursing diploma in the original language.
